Isn’t this normal US healthcare? Insurance will not pay “out of network” (just another greedy invention to fuck over patients) and in network? Well, they usually won’t pay anyway because fuck the sick and elderly, they deserve to die so that rich men get richer; THAT is what’s important in the road now.
On a side note: this insurance CEO should be put against a wall, that is what you do with criminals that cause countless dead and suffering
